In pre-4.0.3 the js was inline (though not from the db), in html spit out by txplib_head.php.
It’s very odd that other external files, like images, are not getting dropped, just the javascript one. That’s what brings me back to something intercepting your browser’s request for it.
Some routers have their own in-built software firewalls as well. I know mine does, it’s a setting I can turn on and off from the router manager.

Just to confirm this from Zem, I got so annoyed with ZA Pro injecting it’s own JS into pages that I turned it’s Ad-blocking features off.
