RFC: merge r4681 and r5075 changes into 4.5.x

It looks like the changes made in r4681 weren’t merged into the 4.5 branch on Google Code, just 4.x. Subsequent changes in r5075 also went into 4.x. Does the introduction of LOG_REFERER_PROTOCOLS fall outside of the major.minor.patch numbering scheme, or could this be merged into the GitHub repo for an upcoming 4.5.x release, should that be in the pipes? I’m still getting the odd-looking referrers in 4.5.7 logs and occasional confused client calls as a result.
I can raise the PR if it’s appropriate.
