Is there a plugin that allows me to create a form as well as upload a file at the same time?

Hmm..I think he is just working on a file uploader. I’m looking for a file uploader and form creation script. Basically, Zem Contact Reborn + the ability to upload a file.

To my knowledge there is no such plugin.

AFU does have a single text field. See it in action on http://www.neme.org/15/submit-a-text.

I agree that we need a more flexible file upload script.

sadlly no. i’ve seen uploadify before but it only uploads files. same goes for the other link too.

You can use uploadify within a form to upload a file and return the filename back to the form for submission as a normal form field.
