default page template - output_form

looking at the default page template, i think this could be improved in a way to help teach newcomers about the system a bit better.

for example in the txp solutions book, there are a lot of code examples which suggest you bundle your main elements of the page into <txp:output_form> tags (header, sidebar, etc).

the default page template should be very concise, with some comments in there to clarify what’s going on. just my opinion of something that can be improved.
