Error page doesn't work

I noticed today that my 404 error page doesn’t come up anymore when traveling to a page or file that doesn’t exist on my site, www.theturninggate.net. I don’t know how long this has been happening. Does anyone know how to get it going again?

Do you still have an “error_default” or “error_404” page template(s)?

Yes, I have an error_default page template.

Does your host allow for custom 404 pages?

Does the diagnostics tab indicate that clean URLs don’t work on this website?

Custom 404 pages used to work; not sure how that was changed. Clean URLs are good.
