[wiki] Add new discussion to talk pages

What is the best way to add a discussion to talk pages. What do you think about ? Click on “+” (creates a new section in the talk page) or just edit the entire page and add content ?
Does a new section (click on “+”) automatically be add below or above the previous section ?
