Form with conditions and file upload

Hello,
a customer asked me for a page with a form, where you can choose who to direct the message to, and where you can also upload a pdf file.
I gave a look to mem_form, without managing to make it work.
Any suggestions how to make the page?

I used Mach Form for a similar situation. A Textpattern plugin method would be great though.
