Improve install screen - fields for mysql connection setup

I don’t think that my version is much better, but i think login and password should be one under another – it’s much more logic and expectevly than currently. And database name referes to login/pass, so i placed it to the top of list.
- I hope my version is more elegant, despite it’s single-column :) The only thing i wory – repeating of “MySQL” in every field. May be this can be deleted?
- May be some notices about fields can be placed next to them, as it is done for Table prefix?
- As for table prefix, may be you should place example of prefix? …, i.e. use newsite_ prefix
