Simple contact form

I want to have a simple contact form on my site. I have done this in PHP before, but I am unsure how to connect this into Textpattern. What is the method for doing an include file to my php file – or is there an easier way to do this – a plugin perhaps? I just want to do a simple contact form with name, email, and message.
