Version 4.0.6
Using hak_tinymce 0.7.4. One of our clients actually reported the error. I have not been able to replicate it.

In older versions a similar error used to occur if you went to one of the vertical preview tabs (like HTML), then used the back-button in the browser and saved the article (a new article, not editing an existing one).
