Fix some IIS clean URL issues?

One of the listed changes in the 4.04 post was Fix some IIS clean URL issues. I have one client that wants to stay the course and run on his Windows host. Can anyone point me in the direction of some documentation on how to achieve clean URLs on IIS? Or if anybody has any solutions, could you post it here?

NOTE: I’m trying to achieve this on a shared windows host, uplinkearth.com

Server Info:
PHP version: 5.0.4
MySQL: 4.1.12
Server: Microsoft-IIS/6.0
PHP Server API: isapi
Server OS: Windows NT 5.2
