sgb_url_handler plugin does not work on virtual hosting(fatcow)?

I installed the sgb_url_handler plugin so I can use URLS in the format http://site/section/category/title. It works perfectly on my prototype system on my lan, but when I try it on my hosting provider(in this case, fatcow.com), it installs without problem, but any attempt to use ‘alternate’ urls redirects me to the default page. Unfortunately, changing hosting is not an option.

I don’t understand what could cause this problem-

There do not appear to be any mod_rewrite complications.
PHP and MySql are of similar versions in both places.
The hosting service appears to be using CGI, rather than mod_php. Maybe that’s it?

Has anyone seen this plugin break before?
How did you fix it?
