comments function error

Hello community
Testing the commentfunction of my templates, I got this error.

Fehler im Tag: <txp:comments_form isize="32" msgcols="32" msgrows="4" /> ->  Textpattern Error: Duplicate entry '34b35214b1555ffcc2285bf753688e4c' for key 'PRIMARY' beim Verarbeiten des Bausteins comments_display auf der Seitenvorlage default01_entfaltung

I get the comment-input form double on each site.
I controled templates and all forms, but I didn`t saw any dublicates. I need an advice how to fix that.

Please help me once more.
